Photo Release Form

I grant to RWDSU Local 338 the right to use, reproduce, and publish photographs, video recordings and/or audio recordings of my image, likeness and/or voice without payment, for publicity, illustration, education, or any other purpose and in any manner and medium, with or without my name; and to alter and composite my image, likeness and/or voice without restriction.  I forever release RWDSU Local 338 and its legal representatives and assigns from any and all claims and liability relating to said photographs, video recordings and/or audio recordings, including without limitation claims of privacy.
